Confronting
Diabetes Crisis through Collaborative Management

http://www.healthwebsummit.com/diabetes120909.htm Join Joe Kvedar, MD and invited guest as they discuss how connected health strategies are working to address the growing diabetes health crisis, by allowing providers and patients to work collaboratively to improve control of blood glucose levels in patients with diabetes, including a report on DiabetesConnect, a new practice-based diabetes management program being implemented at Partners HealthCare. Here’s what connected health expert Joe Kvedar, MD, had to say about connected health strategies and how these programs can apply to better manage diabetes care: “Payment reform will be a catalyst to transform our broken health care system, but without a more well-rounded health information technology strategy – efforts to date have focused chiefly on provider adoption of electronic medical records – providers will not have the tools to truly move to the population-based, health-oriented system that current policy makers envision." "The key to solving the cost and quality dilemma is to employ strategies that involve the patient more in his/her health care decision making. The idea is to offload as much medical decision-making as possible from providers. They are a scarce commodity and expensive labor. A strategy highly focused on physician EMR adoption will not achieve this goal. Rather, another set of tools is required. We call this set of tools ‘connected health’."
