A growing shortage of primary care physicians and the growing prevalence of chronic disease has accelerated demand for a new model of care delivery. The Medical Home, as a new model for primary care, has emerged, evolved and gained considerable traction, even being embedded in Medicare pilot programs enabled under the Tax Relief and Health Care Reform Act of 2006. The new Medical Home model has significant implications for primary care delivery and compensation, chronic care management, evidence based medicine, personal health records, and related initiatives.
