Led by nationally recognized thought leader Paul Keckley, PhD, this session will explore the Medical Home model in depth with respect to trends and issues driving the need and demand for such disruptive innovation; the components, critical features, stakeholders and key topics relating to the model; the impact of the model on chronic care management and major stakeholders; and financial considerations of the model. William DeMarco, a national expert on healthcare delivery system redesign and transformation for provider organizations, health plans and hospitals, will expand upon the strategic, financial and operational implications for these organizations.
A growing shortage
of primary care physicians and the growing prevalence of chronic disease has
accelerated demand for a new model of care delivery. The Medical Home, as a new
model for primary care, has emerged, evolved and gained considerable traction,
even being embedded in Medicare pilot programs enabled under the Tax Relief and
Health Care Reform Act of 2006. The new Medical Home model has significant
implications for primary care delivery and compensation, chronic care
management, evidence based medicine, personal health records, and related
initiatives.

Paul H.
Keckley, Ph.D., is the executive director of the Deloitte Center for
Health Solutions (the Center), a part of Deloitte LLP (Deloitte). He
provides strategic guidance on the development of Center research
and thought leadership. Paul has had 30 years of experience in
academic medicine and the private sector. He is well-regarded for
his knowledge of health care economics, health policy and trend
analysis. Prior to joining Deloitte, Paul served in several key roles at Vanderbilt University. He was the executive director of the Vanderbilt Center for Evidence-based Medicine (VCEBM), associate professor at Vanderbilt University School of Medicine and an associate professor of Health Management at the Owen Graduate School of Business. He oversaw many clinical outsourcing ventures involving care team training, clinical information technology, data management, and evidence-based pathway construction and integration. Paul also has engaged in several joint ventures, including specialty hospitals in the United Kingdom, development of primary care delivery systems in Botswana, partnerships with two Fortune 100 companies pursuing innovative approaches to chronic care management and others that are in early-stage development. |

J. DeMarco MA CMC is the President & Chief Executive Officer of DeMarco
& Associates, Inc., a national, independent healthcare consulting firm
specializing in healthcare delivery system redesign and transformation. The firm specializes in
research and strategy development, organization development of successful
provider owned enterprises and
marketing management including product design and promotion. Mr. DeMarco is recognized
as a leader in the research, design and implementation of community based healthplans. Since his involvement in several startup health plans in
Minnesota in the early 1970s Bill and his team of management consultants
clinical specialists and reimbursement analysts have assisted employers and
physicians in developing better relationships with insurers up to and
including developing local solutions to deliver and finance care. He
is currently involved with assisting special needs plan startups and
